Procedures
- The applicant shall visit Health Professions Licensing Department, fill out the designated application and submit the enclosed documents.
- The documents are verified and the application form is approved.
- The applicant pays the due fees in the Financial Department.
- The data of the applicant will be entered to the Professional License Issuing Program.
- The professional license is issued.
- The Minister of Health or his/her representative signs the license.
- The applicant receives the Professional License.
Required Documents
- An approved and certified copy of the General Certificate of Secondary Education/ scientific or nursing stream.
- A copy of the training certificate stating that the applicant has participated in the rescue of 150 cases.
- An approved copy of 28 weeks theory and practice training certificate in ambulance.
- A non-criminal record certificate.
- A copy of the Civil Status ID card.
- Two (2) personal photographs.

Holders of General Certificate of Secondary Education/ scientific or nursing stream.
